We apologize, but we are a "self-pay" practice. This means we DO NOT accept/bill any forms of insurance including, but not limited to: Worker's Comp, Personal Injury, or Medicare/Medicaid. We do however accept cash, check, major credit/debit cards, and HSA.
If you would like to cancel an appointment, we ask that you provide us with at least 24 hours notice. A fee may be charged for a no show or late cancellation, and patients may lose the privilege of scheduling for repeatedly missing appointments.
